Replacement Certificate

Please fill out the form as detailed and as specific as possible. Starting March 1st, 2023 we will charge $10 for all replacement certificates that we have to mail. You can pick it up for free by appointment only.

Please understand that you lost your certificate. And we are providing an additional service, for free, of replacing the certificate. We will try to be as quick as we can, but we cannot stop all of our other tasks for this one.
